The Practice Area
We mainly cover the areas of Liverpool 3, 7, 8 and 15.
Although, patients who are already registered with the practice and move to nearby districts, such as L6, L17, L18 and L25, may continue with the practice if they wish to do so, after a discussion with the Practice Manager on a case-by-case basis.
To register with us you must complete the following steps:
1. Complete the registration forms provided by the practice.
2. Provide (a) Photographic Identification Documents such as Driving License (UK) or Valid Passport and (b) Utility Bill or Bank Statement with your name & full address.
3. Make an appointment with our practice nurse for New Patient Health Check.
If you are registering your family you will need to complete a separate form for each family member.
All the above instructions are important to keep your records with us as accurate and as up-to-date as possible.
Therefore, it is important that you keep us informed of any changes to your personal details such as your name, date of birth, current address and telephone number(s).
